Student Privacy and Release of "Directory Information"

If a student has attained the age of 18 years or is attending an institution of postsecondary education (including K-12 students simultaneously enrolled in Butte College courses), the rights to access records shall therefore only be required of, and accorded to, the student.

The following “Directory Information” may be released without student consent: student’s name, participation in officially recognized activities and sports, weight and height of members of athletic teams, degrees and awards received. The college also reserves the right to include information about expulsion on student transcripts. Students who do not want directory information released must inform the Office of Admissions and Records in writing on the appropriate form at the beginning of each term the information is to be withheld.  Please email admissions@hklyan.com to request the form.

Forms

Application:

• ESL/Spanish Application PDF – For students applying to take English as a Second Language courses.

Changes to Student Records:

• Change of Information PDF – For students making corrections to their SSN, DOB, or address.  *Some changes can be made in the student Self-Service portal. See included instructions.
• Chosen Name-Legal Name Change PDF – For students changing their Legal or Chosen name.

Registration:

• Audit PDF – For students requesting enrollment to audit a course. *Requires Instructor and Dean signatures.
• Late Drop Petition PDF – For faculty submitting requests for a Late Drop.  *Must be submitted by Instructor or Department Chair.
• Loss of Priority Form PDF - To appeal loss of priority status.
• Pass / No Pass PDF – For students changing grading option to Pass/No Pass.  *Students are encouraged to speak to an academic counselor before requesting this change.
• Refund Request PDF – For students requesting payment for refunds. *Submit to Business Services in SAS 310.

Residency:

• Non-Resident Tuition Waiver (AB540) PDF – For students who qualify for this exemption of Nonresident Tuition.  *See Residency page on the left for more information.
• Residency Re-evaluation PDF – For students requesting their nonresident status to be re-evaluated. 

Transcript Evaluations:

• Prerequisite Evaluation Form PDF – For students requesting clearance of a course prerequisite.
• Prerequisite Petition Challenge Form PDF – For students challenging a course prerequisite or corequisite requirement.
• Transcript Evaluation Request PDF – For current students requesting other college transcripts to be evaluated for credit at Butte College.

Transcript Requests:

• Current K-12 Transcript Request PDF – For current elementary, middle, and high-school students enrolled in courses through Butte College. 

Verifications:

• SARA Evaluation of Physical Location PDF – For students enrolled in only online courses who are required to provide proof of physical location in California.
• Enrollment Verification Request PDF – For students requesting proof of enrollment or non-enrollment.
